LIABILITY WAIVER AND RELEASE

I, the undersigned parent/guardian, acknowledge that participation in basketball activities involves inherent risks including, but not limited to, physical injury. I hereby release, waive, discharge, and covenant not to sue New Kent Basketball Academy (NKBA), its directors, coaches, volunteers, and staff from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by the participant while participating in NKBA activities or while on the premises used for such activities. I agree to indemnify and hold harmless NKBA from any loss, liability, damage, or cost it may incur due to the participation of the minor in NKBA activities. I certify that the participant is physically fit to participate in basketball activities and has no medical conditions that would prevent participation except as disclosed above. I give permission for NKBA staff to authorize emergency medical treatment if necessary.
